Check the courses section below. 📝 Waivers and Forms: What You Need to Know Depending on your situation, the Wisconsin Department of Transportation may require you to obtain an MV3575 Proof of Enrollment Waiver before getting a motorcycle temporary permit. You’ll need this waiver if you: Are under 18 years old Have received three temporary permits from the DMV Have failed the DMV’s motorcycle skills test twice How to Request a Waiver: Waivers are issued after course enrollment. Requests must be made at the Kenosha Campus or by calling 262-564-3900. If you fail to attend your course after receiving a waiver, the DMV will be notified, so make sure to show up and ride on! Tip for Students Under 18: Bring a parent or guardian on the first in-person training day to sign the required "Personal Enrichment" form.
